DIPLOMATE STATUS FOR PHYSICIANS REGISTERED UNDER THE GRANDFATHER CLAUSE

Initial Status – Diplomate Grandfather Clause

Time restriction: Limited time

Cost – $400

Notice: The board shall issue a limited number of Diplomates to applicants who have satisfied the Diplomate Grandfather Clause requirements listed below.

1. Applicant has completely and accurately completed the Diplomate Application provided
2. Applicant provided a signed affidavit to the Board demonstrating that the applicant has engaged in the practice of functional medicine and/or clinical nutrition for at least 300 hours prior to applying.
3. Applicant has paid the application fee in full.    
4. Applicant has met the credential requirements:  

Professional Education - Candidate must possess a doctorate level of education in field of healthcare from a regionally accredited institution of higher learning. Nurse Practitioners and Physician Assistants do not require a doctorate, but they must have successfully completed their medical education from a regionally accredited institution of higher learning.

Licensure - Candidate must possess an active, unrestricted license to practice within their scope.

Good Standing - Candidate must be in good standing from the ABFM and their professional licensure board. Physicians must not have been convicted (or be in the process of being investigated) for a criminal offense.

A Diplomate status issued through this Grandfather Clause is valid and is subject to the same renewal requirements and renewal fees as any other Diplomate issued by the ABFM.
